You will not be denied services because of your inability to pay. We work with most insurance and have discounted fees and financial assistance for anyone who needs it. A sliding-scale fee is available based on family size and income.

Burrell Behavioral Health serves all patients and has discounted fees for those who qualify. In order to qualify for any type of financial assistance, clients or legal guardians of clients are responsible for providing financial information and proof of residency to assist Burrell in determining level of financial assistance. If approved, the client will be able to receive financial assistance for six months. Help with the financial assistance application will be available as a part of the intake process.

Proof of residency includes:

A valid driver’s license
Posted mail with the name of the applicant
Utility bill or mortgage/rental invoice
Voter ID card

Financial information includes one of the following:

Two most recent paystubs
The prior year’s tax return (if reflective of current income)
Statement of Benefits (Social Security, Unemployment, Pensions, etc.)
Employer earnings statements
Federal or State award letter

Types of Financial Assistance Available:

Purchase of Service (POS)

POS funds are state funds available if the client does not have Medicaid or another pay source. For both Recovery and CPR clients, the Community Support Specialist (CSS) will assist the client in applying for Medicaid coverage and POS funds will cover services for six months or until the application is approved. If the Medicaid application is not approved, the CSS will work with the client to find another payment option after the six-month period ends.

Eligible Services:

  • Recovery Services (Substance-Use Disorder Services)
  • Community Psychosocial Rehab (CPR) -- while individual is applying for Medicaid


Sliding Scale Fee

Burrell Behavioral Health’s Sliding Fee Scale is based on a percentage of the Cost of Services. Clients who qualify for the sliding scale fee program for services will be placed into one of six categories depending on their family size and gross household income:

1. Slide A- 100% or less of Federal Poverty Guidelines
2. Slide B- 101% to 125% of Federal Poverty Guidelines
3. Slide C- 126% - 150% of Federal Poverty Guidelines
4. Slide D- 151% to 175% of Federal Poverty Guidelines
5. Slide E- 176% to 200% of Federal Poverty Guidelines
6. Clients over 200% of FPG are Full Fee.

Outpatient Financial Aid

Outpatient Financial Aid is available for clients who qualify for the lowest rate on the sliding scale, but are unable to pay for services. These clients will be able to complete a Financial Aid Agreement Form and, if approved, will receive six (6) sessions at no cost. These sessions will expire if the client fails to show to their appointments without cancelling in advance. Additional sessions may be made available in special cases.

Eligible Services:

  • Outpatient Therapy (Note: Outpatient Financial Aid does not apply to psychiatry, Autism services or occupational therapy


Medication Assistance

If you need help paying for your medication, Burrell Pharmacy staff will supply information about financial assistance provided by Pharmaceutical Companies.

Full Pay

Self-pay clients who pay the full fee at the time of service are eligible for a 20 percent discount.

Your Right to a Good Faith Estimate (GFE)

A GFE will explain how much your medical care will cost when you don’t have insurance or when receiving care from an out-of-network provider. Please note the following:

  • You will receive the total expected cost of any non-emergency items or services
  • Make sure you receive a GFE at least one business day before your scheduled services
  • If you receive a bill that is at least $400 more than your GFE, you can dispute the bill
  • Make sure to save a copy or picture of your GFE
  • No balance billing for out-of-network providers
